We are looking for a detail-oriented, compassionate Registered Nurse to manage the health and well-being of our patients. The Registered Nurse must be well-versed in the administration of highly complex medical conditions, including cardiology, home health and wound care. This position is also responsible for scheduling employee appointments and overseeing internal patient transfers as well as additional administrative duties.

Responsibilities:

• Communicate with patients and families to determine individualized health care plans and goals.
• Determine an individualized health care plan for patients.
• Work with physicians to implement an individualized health care plan.
• Assess members' health status, needs, and concerns.
• Monitor members' conditions for changes and take action as necessary.
• Work with other staff to create a comfortable environment for members.

Job Types: Full-time, Part-time, Per diem

Pay: $38.00 - $100.00 per hour
